Purpose of the Study:

  • To evaluate the performance of bedside sonography in identifying procedure-related pneumothorax.

Main Methods:

  • 185 patients undergoing thoracentesis, transbronchial biopsy, or CT-guided lung biopsy were studied.
  • Pre- and post-procedure transthoracic sonography and post-procedure chest radiography were performed.
  • Chest radiography served as the gold standard for pneumothorax diagnosis.

Main Results:

  • Pneumothorax occurred in 4.0% of patients, primarily after CT-guided needle lung biopsy.
  • Sonography demonstrated 88% sensitivity and 97% specificity for pneumothorax detection.
  • Diagnostic accuracy was 97% for adequate sonographic scans; limited scans in 43 patients showed a likelihood ratio of 1.08.

Conclusions:

  • High-quality bedside chest sonography is a valuable tool for post-procedure pneumothorax evaluation.
  • Chest radiography is advised for patients with pre-existing lung disease that limits sonographic image quality.
