Rab GTPases act in a regulated cascade during membrane fusion, helping the lipid bilayers mix. The Rab family of proteins are active when bound to GTP, and inactive when bound to GDP. Hence, they act as guanine nucleotide-dependent molecular switches. Rab-GTP recognizes and binds to long or short-range tethering proteins to capture the target vesicle. These tethers coordinate with SNAREs on the vesicle and the target membrane to assemble the trans SNARE complex that locks the mixing bilayers.

Summary

A new ultrashort balloon effectively recrosses previously deployed stent struts in coronary bifurcations, improving procedural success and side branch ostial expansion. This addresses challenges in provisional stenting, reducing complications.

Background:

  • Provisional stenting is standard for coronary bifurcations, but routine side branch (SB) ostium dilatation remains debated.
  • Recrossing stent struts with balloons can be difficult, often due to balloon tip obstruction by stent struts.

Purpose of the Study:

  • To evaluate the crossing ability and procedural outcomes of a novel, dedicated ultrashort balloon (Glider) for SB dilatation.
  • To assess the Glider balloon's efficacy as a primary choice versus a bailout strategy for challenging SB ostial recrossing.

Main Methods:

  • A prospective, multicenter international registry included 125 patients (131 bifurcation lesions).
  • The Glider balloon was used either primarily (72%) or as a bailout (28%) after failed attempts with conventional balloons.
  • Technical and procedural success rates, postprocedural dissections, and in-hospital Major Adverse Cardiovascular Events (MACE) were evaluated.

Main Results:

  • Technical success was 92% in the primary group and 83% in the bailout group.
  • Overall procedural success was high at 98%, with the Glider demonstrating superior crossing ability in difficult cases.
  • Complications included ostial SB dissections (9), stent loss (1), coronary spasms (2), and thrombus formation (2).

Conclusions:

  • The Glider's unique beveled tip design facilitates effective recrossing of stent struts when conventional balloons fail.
  • This dedicated balloon improves SB ostial expansion, potentially reducing strut malapposition during provisional bifurcation stenting.
