    Background:

    • Morphological brain alterations are implicated in obsessive-compulsive disorder (OCD) pathophysiology.
    • Limited knowledge exists regarding the morphological connectivity of structural brain networks in OCD and their heritability.

    Purpose of the Study:

    • To investigate the morphological connectivity properties of structural brain networks in individuals with OCD.
    • To explore the heritability of these morphological connectivity properties by comparing patients, siblings, and controls.

    Main Methods:

    • Constructed structural brain networks for three groups: OCD patients (30), siblings (19), and matched controls (30).
    • Utilized 68 cortical regions per subject to define each group's brain network.
    • Measured small-worldness and modularity to assess morphological connectivity.

    Main Results:

    • OCD patients' structural brain networks exhibited atypical small-worldness and modularity compared to controls.
    • Observed decreased local efficiency in small-worldness and altered modularity (reduced intra-connectivity in Module III, increased interconnectivity between Modules I and II).
    • Unaffected siblings displayed similar atypical small-worldness and modularity patterns as OCD patients.

    Conclusions:

    • Atypical small-worldness and modularity in structural brain networks are characteristic of OCD.
    • The similar network properties in unaffected siblings suggest these alterations may represent a candidate endophenotype for OCD.
    • These findings highlight potential genetic underpinnings of brain network morphology in OCD.
