Summary

Newly formed coronary collateral vessels remain functional after long-term reperfusion. Restoring regional myocardial function in these collateral-dependent zones requires six to eight minutes following coronary reocclusion.

Background:

  • Coronary collateral circulation plays a vital role in myocardial protection during ischemia.
  • The long-term patency and functional significance of newly developed collateral vessels after reperfusion are not fully understood.

Purpose of the Study:

  • To investigate the functional status of newly developed coronary collateral vessels after a prolonged period of reperfusion.
  • To determine the time course of functional recovery in collateral-dependent myocardial regions upon reocclusion.

Main Methods:

  • Conscious dogs underwent prolonged reperfusion of the left circumflex coronary artery (LCCA) for 1-15 weeks.
  • Subsequent short-term occlusions (1-2 minutes) were performed to assess regional myocardial function.
  • Regional myocardial shortening was measured to evaluate functional changes.

Main Results:

  • Reocclusion after long-term reperfusion caused significant deterioration in subendocardial segment shortening.
  • Regional myocardial function gradually recovered within 6-8 minutes of occlusion.
  • Long-term reperfusion did not alter the time course of functional recovery during reocclusion.

Conclusions:

  • Newly developed coronary collateral vessels remain significant blood conduits even after extended reperfusion periods.
  • Restoration of regional myocardial function in collateral-dependent areas following coronary reocclusion takes approximately 6-8 minutes.
