Summary

Nocardia asteroides complex caused severe necrotizing scleritis in a contact lens wearer. Prompt treatment with antibiotics successfully resolved the infection, restoring vision.

  • A 52-year-old male contact lens wearer presented with a month of progressive right eye pain and redness.
  • Previous evaluations for autoimmune and infectious causes were inconclusive.
  • Ocular examination revealed necrotizing scleritis without keratitis.

Findings:

  • Scleral culture identified heavy growth of Nocardia asteroides complex.
  • The patient was treated with topical amikacin and oral trimethoprim-sulfamethoxazole (Bactrim).
  • Complete resolution of the infection and return to baseline visual acuity were achieved after several weeks of treatment.
