Background:

Swedish valuation sets for EQ-5D (EuroQol 5-Dimensions) health states are currently lacking.
Experience-based health state valuations are increasingly advocated for economic evaluations.
Swedish reimbursement guidelines recommend experience-based values for Quality-Adjusted Life Year (QALY) calculations.

Purpose of the Study:

To estimate Swedish experience-based value sets for EQ-5D health states.
To model the association between Time Trade-Off (TTO) and Visual Analogue Scale (VAS) values and EQ-5D dimensions using population survey data.

Main Methods:

Approximately 45,000 individuals completed the EQ-5D questionnaire and provided TTO and VAS valuations of their health.
Regression models were used to associate TTO and VAS values with EQ-5D dimensions and severity levels.
The association between TTO/VAS values and self-rated health (SRH) severity levels was assessed.

Main Results:

Dimensions, except usual activity, had less impact on TTO valuations compared to hypothetical UK values.
Anxiety/depression demonstrated the greatest impact on both TTO and VAS valuations.
TTO and VAS values were consistently related to SRH; demographic factors (age, sex, education, socioeconomic group) had a modest effect.

Conclusions:

This study provides essential Swedish experience-based value sets for EQ-5D.
The significant impact of anxiety/depression underscores the importance of mental health in overall Health-Related Quality of Life (HRQoL).
Utilizing population surveys aligns with current best practices and may mitigate biases found in hypothetical valuation studies.
