Vestibular symptoms in chronic otitis media (COM) can be assessed using vestibular-evoked myogenic potential (VEMP) tests. These tests, alongside audiometry, help detect inner ear deficits in COM patients.

Background:

  • Vestibular symptoms in chronic otitis media (COM) are challenging to investigate due to contraindications for standard caloric tests in perforated ears.
  • Evaluating inner ear function in COM patients requires alternative diagnostic methods.

Purpose of the Study:

  • To assess inner ear deficits in patients with COM using audiometry, ocular vestibular-evoked myogenic potential (oVEMP), and cervical VEMP (cVEMP) tests.
  • To determine the sequence of inner ear dysfunction in COM.

Main Methods:

  • Audiometry, ocular vestibular-evoked myogenic potential (oVEMP), and cervical VEMP (cVEMP) tests were performed on 85 COM patients (117 ears).
  • Otoscopy and imaging studies were also conducted.
  • Bone-conducted (BC) hearing thresholds were analyzed.

Main Results:

  • Abnormal BC hearing thresholds were found in 37% of ears.
  • Abnormalities were observed in 65% of cVEMP tests, 62% of oVEMP tests, and 37% of BC hearing thresholds, indicating a decline in inner ear function.
  • cVEMP and oVEMP results correlated significantly with BC hearing thresholds.

Conclusions:

  • The sequence of inner ear deficits in COM progresses from the saccule/utricle to the cochlea and semicircular canals.
  • cVEMP and oVEMP tests, in addition to BC hearing tests, can aid in the early detection of inner ear involvement in COM patients.
