Area of Science:

  • Ophthalmology
  • Neurology
  • Toxicology

Background:

  • Dilated pupils (mydriasis) present a diagnostic challenge for clinicians.
  • Understanding the causes of mydriasis is crucial for accurate diagnosis and patient management.

Purpose of the Study:

  • To review current data and guidelines on the evaluation of dilated pupils.
  • To provide a comprehensive overview of mydriasis causes and diagnostic approaches.

Main Methods:

  • Literature review of recent reports and clinical guidelines.
  • Analysis of etiological factors contributing to pupillary dilatation.

Main Results:

  • Mydriasis can result from topical synthetic parasympatholytic agents and contaminated antihistamines.

  • Atropine, scopolamine, and tropane alkaloids from Datura plants are identified causes.
  • Causes can be unilateral, bilateral, and transient, necessitating careful assessment.
  • Conclusions:

    • A structured, stepwise clinical evaluation is essential for diagnosing the cause of dilated pupils.
    • Appropriate diagnostic pathways can prevent unnecessary and costly investigations.
