    Diagnosing glaucoma is challenging when optic nerve head drusen are present. This co-occurrence complicates the assessment of visual field defects and optic nerve appearance, requiring careful monitoring.

    Background:

    • Glaucoma and optic nerve head drusen can coexist, complicating diagnosis.
    • Pigmentary glaucoma and pigmentary dispersion syndrome are frequently associated with this comorbidity.

    Observation:

    • Five patients with both optic nerve head drusen and glaucoma were studied.
    • Four of the five patients had pigmentary glaucoma or pigmentary dispersion syndrome.

    Findings:

    • Optic nerve head drusen do not prevent glaucoma-related cupping.
    • Drusen interfere with the accurate interpretation of optic nerve head cupping and visual field defects.

    Implications:

    • Management necessitates regular visual field testing and optic disc imaging.
    • Maintaining low intraocular pressure is crucial for patients with coexisting glaucoma and optic nerve head drusen.
