This case report details a rare instance of unilateral choroidal neovascularization (CNV) in a young myopic male, secondary to punctate inner choroidopathy. Treatment with bevacizumab improved both vision and ocular anatomy.

  • A 29-year-old Caucasian male presented with acute, unilateral decreased central vision in his left eye.
  • Fundoscopy revealed macular lesions, and OCT showed retinal thickening and detachments.
  • Fluorescein angiography demonstrated hyperfluorescent lesions indicative of CNV.

Findings:

  • The patient was diagnosed with unilateral CNV secondary to punctate inner choroidopathy.
  • Intravitreal bevacizumab injection led to significant anatomic and visual recovery in the affected eye.

Implications:

  • This case highlights the importance of considering CNV secondary to punctate inner choroidopathy in young myopic patients with visual disturbances.
  • Early diagnosis and treatment with anti-VEGF agents like bevacizumab can restore vision in such rare cases.
