Background:

Purpose of the Study:

  • To assess if a dual approach using Clinical Reasoning Problems (CRP) and Script Concordance Tests (SCT) provides a valid, reliable, and targeted analysis of clinical reasoning.
  • To determine if this combined method can facilitate the development of diagnostic thinking in medical students.

Main Methods:

  • Three participant groups (GPs, final-year medical students) completed 10 CRP and 10 SCT online clinical scenarios.
  • Scores were analyzed for reliability, inter-format correlation, and differences across expertise levels.

Main Results:

  • Modest reliability coefficients (Cronbach's alpha 0.36-0.61) were observed for both SCT and CRP.
  • Significant correlations were found between CRP and SCT scores, indicating overlapping but distinct measurement characteristics.
  • Expertise-based differences in scores were noted, particularly for CRPs.

Conclusions:

  • Clinical Reasoning Problems (CRP) and Script Concordance Tests (SCT) are complementary measures of clinical reasoning.
  • Using both CRP and SCT provides a more thorough appraisal of clinical skills than either alone, enabling customized teaching.
  • Further research is needed to enhance reliability and confirm suitability for assessment.
