A new technique effectively prevents inguinal hernias after radical retropubic prostatectomy (RRP). This simple procedure significantly reduces hernia incidence, offering a high hernia-free survival rate for patients undergoing RRP.

Area of Science:

  • Urology
  • Surgical Innovation

Background:

  • Inguinal hernias are a common complication following radical retropubic prostatectomy (RRP).
  • Existing methods for prevention may be complex or time-consuming.

Purpose of the Study:

  • To introduce and evaluate a novel, simple technique for preventing inguinal hernias concurrently with RRP.
  • To assess the efficacy of this new procedure in reducing postoperative inguinal hernia rates.

Main Methods:

  • A prospective study involving 230 Japanese prostate cancer patients undergoing RRP.
  • 115 patients received the novel hernia prevention procedure alongside RRP, involving manipulation of the vas deferens and spermatic vessels.
  • 115 patients served as a control group without the prevention procedure.

Main Results:

  • The control group had an inguinal hernia incidence of 15.7% (18/115) with hernia-free survival rates of 89.6% and 84.1% at 1 and 2 years post-surgery.
  • The group undergoing the prevention procedure experienced only a 0.87% incidence of inguinal hernia (1/115).
  • Hernia-free survival rates were 100% at both 1 and 2 years post-surgery in the intervention group (P<0.0001).

Conclusions:

  • A simple, easily performable technique for preventing post-RRP inguinal hernias has been developed.
  • This novel procedure demonstrates excellent outcomes and significantly reduces the incidence of inguinal hernias after RRP.
