Purpose of the Study:

  • To determine which characteristics of peer coaches are linked to better diabetes control in low-income patients with type 2 diabetes.
  • To explore the association between coach self-efficacy, distress, and depression with patient glycosylated hemoglobin (A1C) improvement.

Main Methods:

  • A 6-month peer coaching intervention was conducted with 20 trained peer coaches and 109 patients with type 2 diabetes.
  • Data collected on peer coach characteristics: age, gender, diabetes duration, A1C, BMI, diabetes distress, self-efficacy, and depression.
  • Primary outcome: average change in patient glycosylated hemoglobin (A1C).

Main Results:

  • Patient A1C improvement was associated with coaches having lower self-efficacy in diabetes management (P < .001).
  • Higher levels of coach-reported diabetes-related distress (P = .01) and lower depression scores (P = .03) were also linked to improved patient A1C.
  • Conversely, higher coach self-efficacy and lower distress were not associated with better patient outcomes.

Conclusions:

  • Specific peer coach characteristics, such as lower self-efficacy and higher distress, are unexpectedly associated with improved patient A1C.
  • Coach uncertainty regarding their own diabetes management may enhance patient self-management.
  • These findings suggest a need to reconsider traditional metrics for selecting peer coaches in diabetes interventions.
