    Pacemakers can sometimes miss P waves, leading to dropped beats. This study clarifies when this is true atrial undersensing versus normal pacemaker function to prevent unnecessary procedures.

    Background:

    • Atrioventricular (AV) universal (DDD) pacemakers are commonly implanted for cardiac rhythm management.
    • Intermittent dropped P waves on electrocardiogram (ECG) can be a concerning finding in patients with pacemakers.

    Observation:

    • Three patients with DDD pacemakers exhibited intermittent dropped P waves on their ECGs.
    • One patient demonstrated true atrial undersensing.
    • The other two patients had appropriate atrial electrode sensing, but normal pacemaker operation intermittently blocked sinus P wave sensing.

    Findings:

    • Distinguishing true atrial undersensing from intermittent P wave sensing inhibition by normal pacemaker function is crucial.
    • ECG analysis can help differentiate these scenarios.

    Implications:

    • Accurate diagnosis of dropped P waves in DDD pacemaker patients can prevent unnecessary pacemaker reprogramming or replacement.
    • Understanding pacemaker sensing behavior is key to appropriate patient management and avoiding invasive interventions.
