Background:

  • Radial keratotomy (RK) is a refractive surgery procedure that can lead to long-term complications.
  • Some patients who have undergone RK may experience decreasing visual acuity and diurnal visual fluctuations.

Purpose of the Study:

  • To evaluate the outcomes of corneal collagen crosslinking (CXL) in patients with a history of RK presenting with visual disturbances.
  • To assess the safety and efficacy of CXL in restoring corneal stability after RK.

Main Methods:

  • Retrospective chart review of patients who underwent CXL following RK.
  • Data collected included uncorrected and corrected distance visual acuity (CDVA), manifest refraction, and corneal topography preoperatively and up to 12 months post-CXL.

Main Results:

  • Eight out of nine eyes in 6 patients experienced a discontinuation of diurnal visual fluctuations within 6-12 months post-CXL.
  • Mean CDVA improved from 0.2 to 0.1 logMAR, and mean average keratometry decreased from 40.1 to 39.1 diopters, though not statistically significant (P=0.5 and P=0.06).
  • Some positive effects observed at 6 months were slightly reduced by 12 months.

Conclusions:

  • Corneal collagen crosslinking (CXL) is a safe and effective treatment for corneal instability following radial keratotomy (RK).
  • CXL can significantly reduce diurnal visual fluctuations and improve visual acuity in these patients.
  • Longer follow-up is needed to confirm the sustained efficacy of CXL after RK.
