Summary

This case report details a rare instance of hypotropic dissociated vertical deviation (DVD) in a young adult. Surgical correction of esotropia successfully aligned the eyes, with mild amblyopia noted in the non-fixating eye.

Background:

  • Dissociated vertical deviation (DVD) is a rare condition affecting binocular vision.

Observation:

  • A 25-year-old female presented with unilateral esotropia, hypotropia, and a slow downward drift in the left eye.
  • The patient had a history of esotropia since infancy and developed hyperopia.
  • Best corrected visual acuity was 20/20 in the right eye and 20/40 in the left eye.

Findings:

  • Surgical intervention involved bimedial rectus muscle recession for significant esotropia.
  • Post-operatively, the patient achieved orthophoria at 3 months with no further surgical plans for hypotropic DVD.
  • The affected eye exhibited only mild amblyopia, suggesting an acquired etiology likely linked to early-onset accommodative esotropia.

Implications:

  • This case highlights that hypotropic DVD can present with mild amblyopia, even when associated with early-onset esotropia.
  • Acquired hypotropic DVD may be managed effectively with strabismus surgery.
  • Further research into the specific etiologies and long-term outcomes of hypotropic DVD is warranted.
