Nonspecific esterase staining patterns in acute monocytic leukemia.

U Wrotnowski, D J Innes, A S Hobson

    American Journal of Clinical Pathology
    |April 1, 1987

    Summary

Nonspecific esterase (NSE) staining in acute monocytic leukemia shows two patterns: diffuse and focal. Focal NSE activity was observed in all hypocellular leukemia cases, but clinical presentation did not differ between patterns.

    Area of Science:

    • Hematology
    • Oncology
    • Pathology

    Background:

    • Acute monocytic leukemia (M5) is a subtype of acute myeloid leukemia.
    • Nonspecific esterase (NSE) is an enzyme often used in the diagnosis of myeloid leukemias.
    • Variability in NSE staining patterns may correlate with specific leukemia subtypes or clinical features.

    Purpose of the Study:

    • To analyze retrospective NSE staining patterns in acute monocytic leukemia (M5).
    • To correlate NSE staining variability with clinical findings in M5 leukemia.
    • To distinguish between different NSE activity patterns and their association with leukemia cellularity.

    Main Methods:

    • Retrospective analysis of 14 acute monocytic leukemia (M5) cases.
    • Use of alpha-naphthyl acetate as a substrate for NSE staining.
    • Categorization of NSE staining into diffuse fine granular and dense focal patterns.

    Main Results:

    • Two distinct NSE staining patterns were identified: diffuse and focal.
    • Six cases showed diffuse staining (Group 1), all hypercellular acute leukemias.
    • Eight cases showed focal staining (Group 2), including four hypercellular and four hypocellular acute leukemias.
    • All hypocellular leukemia cases exhibited the focal NSE staining pattern.

    Conclusions:

    • NSE staining patterns in acute monocytic leukemia can be categorized as diffuse or focal.
    • Focal NSE activity is a characteristic finding in hypocellular acute monocytic leukemia.
    • Despite distinct staining patterns, no significant differences in clinical presentation or disease course were observed between the groups.
