Background:

  • The National Tuberculosis Control Programme (NTP) in Pakistan reported a 64% TB case detection rate in 2011.
  • An estimated incidence rate of 230 per 100,000 population was noted, but under-reporting, especially in the private sector, was suspected.

Purpose of the Study:

  • To determine the actual tuberculosis (TB) incidence and notification rates in Pakistan for 2012.
  • To assess the completeness of TB case reporting within the national program.

Main Methods:

  • A surveillance system was implemented across public and private healthcare providers in 12 selected districts.
  • Record linkage and capture-recapture analysis were employed to estimate unobserved TB cases.

Main Results:

  • A total of 8,346 TB cases were identified, with 6,061 reported to the NTP.
  • An estimated 10,030 unobserved TB cases were found, indicating only 32% of cases were notified.
  • The calculated annual incidence rate was 497 per 100,000 population, significantly higher than official estimates.

  • The proportion of TB cases notified to the NTP was substantially lower than expected.
  • Actual TB incidence rates in Pakistan are higher than previously estimated.
