Summary

Intravascular brachytherapy (IVBT) improves artery patency and reduces restenosis after angioplasty in peripheral artery disease patients. However, evidence is limited, and more data are needed on long-term outcomes and comparisons with new technologies.

Background:

  • Atherosclerosis causes arterial narrowing, treated by bypass, angioplasty (PTA), or stenting.
  • These treatments have high failure rates within a year.
  • Intravascular brachytherapy (IVBT) uses localized radiation to inhibit restenosis.

Purpose of the Study:

  • To assess the efficacy and complications of IVBT.
  • To evaluate IVBT's role in maintaining artery patency post-angioplasty or stenting.
  • To analyze IVBT for iliac or infrainguinal arteries and bypass grafts.

Main Methods:

  • Systematic review of randomized controlled trials (RCTs).
  • Searched Cochrane Peripheral Vascular Diseases Group Specialised Register and CENTRAL.
  • Included RCTs comparing endovascular treatment with or without IVBT for peripheral arterial disease.

Main Results:

  • Eight trials (1090 participants) focused on femoropopliteal arteries; no iliac artery studies found.
  • IVBT showed higher cumulative patency at 24 months (OR 2.36) and reduced restenosis at 6, 12, and 24 months.
  • Target lesion revascularization was reduced at 6 months (OR 0.51); no significant differences in quality of life or complications were observed.

Conclusions:

  • Limited evidence exists for IVBT in peripheral artery disease due to inconsistent outcome reporting.
  • More data needed on HRQOL, limb salvage, long-term outcomes, and comparisons with drug-eluting stents/balloons.
  • Adequately powered RCTs and cost-effectiveness data are required for widespread recommendation.
