Background:

  • Dysphagia is a common and debilitating symptom in patients with malignant tumors of the esophagus or gastric cardia.

Purpose of the Study:

  • To evaluate the efficacy and safety of Neodymium Yttrium Aluminium Garnet (NdYag) laser treatment for dysphagia caused by malignant esophageal or gastric cardia tumors.
  • To assess the immediate and long-term functional outcomes and survival rates in patients treated with NdYag laser therapy.

Main Methods:

  • Seventy-six patients with malignant esophageal or gastric cardia tumors causing dysphagia underwent NdYag laser treatment.
  • Laser parameters: 80-100 watts power, 0.5-1 second pulses.
  • Average of four treatment sessions per patient, with follow-up endoscopy at four-week intervals.

Main Results:

  • Immediate improvement in swallowing was observed in 95% of patients (32% swallowing freely, 54% solids, 9% liquids).
  • The mortality rate associated with laser recanalization was 5%.
  • Swallowing improvement was maintained until death in 85% of patients; mean survival was 19 weeks.

Conclusions:

  • NdYag laser therapy is an effective palliative treatment for dysphagia in malignant esophageal and gastric cardia tumors.
  • The procedure offers significant functional improvement and maintains swallowing ability for the majority of patients.
  • Laser recanalization provides a valuable option for improving quality of life in advanced stages of these cancers.
