Summary

This case study highlights a successful penile prosthesis implantation in a sickle cell disease patient experiencing recurrent priapism and corpus cavernosum abscess. Early intervention with prosthesis implantation resolved erectile dysfunction and infection, demonstrating a positive outcome.

Background:

Recurrent priapism in sickle cell disease (SCD) can lead to serious complications.
The Winter procedure, a surgical intervention for priapism, can sometimes be complicated by infection, such as corpus cavernosum abscess.

Observation:

A 23-year-old male SCD patient presented with his third priapism episode, complicated by a corpus cavernosum abscess post-Winter procedure.
The patient underwent multiple drainage procedures and antibiotic therapy, leading to clinical improvement of the infection but persistent erectile dysfunction.

Findings:

Despite initial treatments, the patient developed drug-refractory erectile dysfunction.
Early penile prosthesis implantation, combined with vacuum pump therapy, was performed to address fibrosis and erectile dysfunction.

Implications:

This case suggests that early penile prosthesis implantation can be an effective treatment for managing erectile dysfunction secondary to complex priapism complications in SCD patients.
Prompt surgical intervention, including prosthesis placement, may improve functional outcomes and quality of life in these challenging cases.
