This case study highlights a successful penile prosthesis implantation in a sickle cell disease patient experiencing recurrent priapism and corpus cavernosum abscess. Early intervention with prosthesis implantation resolved erectile dysfunction and infection, demonstrating a positive outcome.

Background:

  • Recurrent priapism in sickle cell disease (SCD) can lead to serious complications.
  • The Winter procedure, a surgical intervention for priapism, can sometimes be complicated by infection, such as corpus cavernosum abscess.

Observation:

  • A 23-year-old male SCD patient presented with his third priapism episode, complicated by a corpus cavernosum abscess post-Winter procedure.
  • The patient underwent multiple drainage procedures and antibiotic therapy, leading to clinical improvement of the infection but persistent erectile dysfunction.

Findings:

  • Despite initial treatments, the patient developed drug-refractory erectile dysfunction.
  • Early penile prosthesis implantation, combined with vacuum pump therapy, was performed to address fibrosis and erectile dysfunction.

Implications:

  • This case suggests that early penile prosthesis implantation can be an effective treatment for managing erectile dysfunction secondary to complex priapism complications in SCD patients.
  • Prompt surgical intervention, including prosthesis placement, may improve functional outcomes and quality of life in these challenging cases.
