Sporadic multiple ossifying fibromas (OFs) are rare and challenging to diagnose, requiring differentiation from HPT-JT syndrome. Testing for serum parathyroid hormone (PTH) and HRPT2 mutations is recommended for patients with multiple OFs.

Findings:
- Sporadic multiple OFs constitute 2.0% of OF cases.
- No GNAS or HRPT2 mutations were identified in the sporadic multiple OF cases.
- HPT-JT related OFs showed a higher incidence of multiple jaw lesions (33.3%) compared to sporadic cases.
Implications:
- Early differentiation of sporadic multiple OFs from HPT-JT syndrome is vital for appropriate patient management.
- Serum PTH testing and HRPT2 mutation screening are strongly recommended for patients diagnosed with multiple ossifying fibromas.
