Serum levels of Interleukin-21 (IL-21) did not differ in early rheumatoid arthritis (RA) patients versus controls but decreased with treatment. Lower IL-21 correlated with improved RA disease activity.

Background:

  • Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by joint inflammation.
  • Interleukin-21 (IL-21) is implicated in the pathogenesis of autoimmune diseases, including RA.

Purpose of the Study:

  • To investigate serum IL-21 levels in recent-onset RA patients.
  • To correlate IL-21 levels with disease activity and treatment response in early RA.

Main Methods:

  • Serum IL-21 levels were measured in 51 RA patients (pre- and post-treatment) and 36 healthy controls.
  • Disease activity was assessed using Disease Activity Score for 28 joints, C-reactive protein, and joint counts.
  • Correlations between IL-21 levels, disease activity, and treatment outcomes were analyzed.

Main Results:

  • IL-21 levels were not elevated in early RA patients compared to healthy individuals.
  • Serum IL-21 levels significantly decreased after 12 weeks of treatment.
  • Baseline IL-21 levels correlated with disease activity measures; decreasing IL-21 correlated with improved disease activity.

Conclusions:

  • Circulating IL-21 levels may not be elevated in early RA but decrease with effective treatment.
  • IL-21 levels show potential as a biomarker for disease activity and treatment response in early RA.
