Etanercept successfully treated a child with severe general pustular psoriasis (GPP) and Cushing syndrome when other therapies failed. This biologic offers a safe and effective option for refractory pediatric GPP.
Area of Science:
- Pediatric Dermatology
- Immunodermatology
- Pharmacology
Background:
- General pustular psoriasis (GPP) is a rare, severe pediatric skin condition with serious complications.
- Previous treatments like acitretin, methotrexate, and methylprednisolone were ineffective in this case.
- The patient also had iatrogenic secondary Cushing syndrome, complicating treatment options.
Observation:
- An 8-year-old boy with GPP and Cushing syndrome was treated with etanercept.
- Etanercept targets tumor necrosis factor-alpha (TNF-alpha), a key inflammatory mediator.
- This treatment was administered after failure of conventional therapies.
Findings:
- Etanercept demonstrated significant clinical response in the pediatric GPP patient.
- Long-term efficacy was observed without acute exacerbations.
- The treatment exhibited an excellent safety and tolerability profile.
Implications:
- Etanercept represents a viable therapeutic option for children with refractory GPP.
- This case highlights the potential of TNF-alpha inhibitors in managing severe pediatric psoriasis variants.
- Further research into biologics for pediatric GPP is warranted.
