Summary

The harmonic scalpel (HS) significantly reduces thyroidectomy operative times compared to conventional hemostasis (CH). HS also lowers the incidence of transient hypocalcemia and recurrent laryngeal nerve palsies, improving patient outcomes.

Area of Science:

  • Surgical Oncology
  • Endocrine Surgery
  • Minimally Invasive Surgery

Background:

  • Conventional hemostasis (CH) techniques in thyroidectomy can be time-consuming.
  • Evaluating alternative hemostasis methods is crucial for optimizing surgical procedures and patient recovery.

Purpose of the Study:

  • To compare the operative times and postoperative complication rates of harmonic scalpel (HS) versus conventional hemostasis (CH) in thyroidectomy.
  • To assess the safety and efficacy of HS in thyroid surgery.

Main Methods:

  • A randomized study involving 85 patients undergoing open thyroidectomy.
  • Patients were allocated to either the conventional hemostasis (CH) group or the harmonic scalpel (HS) group.
  • Operative time, blood loss, and postoperative complications were analyzed using statistical tests.

Main Results:

  • The harmonic scalpel (HS) group demonstrated significantly shorter operative times (47.2 min) compared to the conventional hemostasis (CH) group (79.2 min).
  • No significant difference in blood loss or hospital stay was observed between the groups.
  • The HS group had a lower incidence of transient hypocalcemia (16% vs. 48%) and no recurrent laryngeal nerve palsies, versus 4.7% in the CH group.

Conclusions:

  • The harmonic scalpel (HS) is a safe and reliable tool for thyroidectomy, reducing operative time.
  • Utilizing HS in thyroidectomy leads to decreased postoperative pain, drainage, and transient hypocalcemia compared to conventional hemostasis (CH).
  • HS offers potential benefits for patient recovery and surgical efficiency in thyroid procedures.
