Background:

Hernia repair using synthetic mesh is a common surgical procedure.
Mesh erosion into adjacent organs, such as the bladder, is a rare but serious complication.
Recurrent hematuria can be a presenting symptom of mesh migration.

Observation:

Two patients presented with recurrent frank hematuria.
Initial evaluations suggested a malignant tumor.

Findings:

Cystoscopy and surgery revealed hernia mesh migrated into the bladder.
Mesh migration occurred 3-4 years after hernioplasty.

Implications:

This complication can mimic bladder cancer.
Awareness is crucial for accurate diagnosis and management of post-hernioplasty hematuria.
