Summary

Mandibular setback surgery using intraoral vertical subcondylar osteotomy (IVSO) is stable one year post-treatment. This combined surgical and orthodontic approach offers reliable skeletal and dental results for Angle Class III malocclusions.

Purpose of the Study:

  • To assess the effectiveness of intraoral vertical subcondylar osteotomy (IVSO) combined with intermaxillary fixation and orthodontic treatment.
  • To determine the degree of skeletal and dental relapse following this surgical-orthodontic combination.

Main Methods:

  • A retrospective study of 28 patients with Angle Class III malocclusions who underwent IVSO and orthodontic treatment.
  • Cephalometric measurements and dental cast analyses were performed before treatment, 8 weeks post-surgery, and 1 year post-surgery.
  • Statistical analysis was used to evaluate changes in skeletal and dentoalveolar relationships.

Main Results:

  • Small but statistically significant skeletal relapse was observed in sagittal and vertical planes (ANB, Wits appraisal, ML-NSL, Li-NB).

  • No significant dentoalveolar relapse was detected based on dental cast evaluations.

Conclusions:

  • Orthodontic treatment combined with IVSO demonstrates stable dental and skeletal results one year after treatment.
  • The procedure provides predictable outcomes for patients with Angle Class III malocclusions.
  • While minor skeletal relapse can occur, the overall stability is considered good.
