Background:

  • Passenger lymphocyte syndrome (PLS) typically arises from ABO blood group incompatibilities in stem cell or solid organ transplants.

  • A blood group O/Rh-positive patient underwent a stem cell transplant from an A/Rh-negative donor with known anti-D antibodies.
  • Despite plasma reduction and replacement in the stem cell product, the recipient showed signs of hemolysis, including a positive antibody screen and direct antiglobulin test.

  • The patient developed significant hemolysis 8 days post-transplantation.
