    Summary

    Single incision laparoscopic surgery (SILS) offers a minimally invasive approach for total colectomy. This study demonstrates the feasibility and safety of a novel clipless SILS total colectomy procedure.

    Background:

    • Single Incision Laparoscopic Surgery (SILS) is increasingly utilized for various surgical procedures.
    • However, reports on SILS for total colectomy remain limited.

    Purpose of the Study:

    • To describe and evaluate a novel, original procedure for single incision, clipless laparoscopic total colectomy.
    • To assess the feasibility and safety of this technique.

    Main Methods:

    • A 3-cm umbilical incision was used to place a SILS port.
    • The procedure involved division of the greater omentum, takedown of colon flexures, creation of a mesocolon window, and clipless vessel/mesocolon division using LigaSure.
    • Ileorectal anastomosis was performed using a double stapling technique.

    Main Results:

    • Eight consecutive patients underwent the procedure between 2010 and 2012.
    • Mean operative time was 228.1 minutes, with a mean blood loss of 64.0 mL.
    • Mean postoperative hospitalization was 10.8 days, with no reported complications.

    Conclusions:

    • The described single incision, clipless laparoscopic total colectomy procedure appears to be a feasible and safe surgical option.
    • This technique may offer advantages in minimally invasive gastrointestinal surgery.
