Summary

Laparoendoscopic single-site (LESS) retroperitoneoscopic nephrectomy is a feasible and safe approach for giant hydronephrosis. This minimally invasive technique offers an attractive option for selected patients requiring kidney removal.

Background:

  • Giant hydronephrosis often necessitates kidney removal.
  • Conventional retroperitoneal laparoscopic nephrectomy is established but can be technically demanding.
  • Minimally invasive approaches aim to reduce patient morbidity.

Purpose of the Study:

  • To evaluate the feasibility and safety of laparoendoscopic single-site (LESS) retroperitoneoscopic nephrectomy.
  • To assess outcomes for patients with giant hydronephrosis and nonfunctioning kidneys.

Main Methods:

  • 22 patients with giant hydronephrosis underwent LESS retroperitoneoscopic nephrectomy.
  • A single-port device was used with a 2.5-cm incision.
  • Lengthened prebent and conventional laparoscopic instruments were employed.

Main Results:

  • Successful completion in all patients without conversion to open surgery.
  • Mean operative time was 98 minutes, with average blood loss of 45 mL.
  • No major intraoperative complications and an uneventful postoperative course with a mean hospital stay of 3.4 days.

Conclusions:

  • LESS retroperitoneoscopic nephrectomy is technically feasible and safe for selected patients with giant hydronephrosis.
  • The use of combined conventional and prebent laparoscopic instruments is effective for LESS retroperitoneal procedures.
