  • Glandular odontogenic cyst (GOC) is an uncommon developmental odontogenic cyst, accounting for 0.012–1.3% of jaw cysts.
  • GOC is exceptionally rare, with only 111 cases reported in English literature.

Observation:

  • This report details a GOC case in a 40-year-old male, located in the left mandibular region.
  • The cyst extended across the midline, an unusual presentation for this type of lesion.

Findings:

  • GOC frequently occurs in the anterior mandible across a wide age range (mean 49.5 years) and shows a tendency for recurrence.
  • Differential diagnosis is challenging due to overlapping histopathological features with other odontogenic cysts and central low-grade mucoepidermoid carcinoma.

Implications:

  • Accurate histopathological diagnosis is crucial for appropriate management of GOC.
  • This case underscores the importance of considering GOC in the differential diagnosis of mandibular cystic lesions, especially those with atypical presentations.
