Background:

• Accessory breast tissue (ABT) is an uncommon condition affecting 0.4-6% of women.
• It most frequently occurs in the axilla and can present diagnostic challenges, particularly when unilateral and large.

Observation:

• A 32-year-old woman presented with a large right-sided axillary mass.
• The mass was initially clinically diagnosed as a pedunculated lipoma.

Findings:

• Histopathological examination confirmed the mass to be accessory breast tissue.
• The tissue exhibited lactational changes, indicating hormonal influence.

Implications:

• Accurate diagnosis of ectopic breast tissue is vital due to its potential for developing pathologies similar to normal breast tissue, including mastitis, fibrocystic disease, and carcinoma.
• This case highlights the importance of histopathological confirmation for axillary masses that may represent ectopic breast tissue.
