Background:

  • Jejunal pseudodiverticula are rare outpouchings of the intestinal mucosa.
  • Intestinal bleeding from these lesions can be a significant clinical challenge, particularly in the elderly.

Observation:

  • Two cases of elderly female patients presenting with significant intestinal bleeding due to jejunal pseudodiverticula were observed within a six-month period.
  • One patient, aged 91, had multiple pseudodiverticula and was managed conservatively.
  • The second patient, aged 86, with a solitary pseudodiverticulum, required surgical resection due to persistent, intensive bleeding.

Findings:

  • Conservative management was successful for jejunal pseudodiverticula with multiple lesions.
  • Surgical resection effectively controlled severe intestinal hemorrhage from a solitary jejunal pseudodiverticulum.
  • Both elderly patients achieved full recovery and were discharged from the hospital.

Implications:

  • This highlights the importance of considering jejunal pseudodiverticula in the differential diagnosis of obscure gastrointestinal bleeding in the elderly.
  • Management strategies should be individualized based on the number of pseudodiverticula and the severity of bleeding.
  • Further review of literature is warranted to establish optimal treatment guidelines for this rare condition.
