Background:

Mandibular first molars frequently exhibit complex root canal anatomy.
Recent case reports indicate an increasing prevalence of variations exceeding four root canals.

Observation:

A mandibular first molar presented with an unusual six root canal systems: three in the mesial root and three in the distal root.
The root canal morphology was classified as Sert and Bayirli type XV in both roots.

Findings:

Successful non-surgical endodontic management was achieved for the mandibular first molar with six canals.
The tooth was restored with a resin composite core and a porcelain-fused-to-metal crown.

Implications:

Clinicians must exercise caution and thoroughness in endodontic treatment of mandibular first molars.
Radiographic interpretation should be considered a preliminary guide, not definitive for complex root anatomy.
This case underscores the significance of recognizing and managing rare morphological deviations in endodontics.
