Summary

Combined anticoagulation and antiplatelet therapy is effective for arteriovenous embolism. This treatment approach improved prognosis in a patient with coexisting venous thromboembolism and arterial thrombosis, without major bleeding.

  • Arteriovenous embolism, a rare condition involving both venous thromboembolism and arterial thrombosis, presents significant clinical challenges.
  • Simultaneous occurrence of venous and arterial thrombosis necessitates careful management to balance ischemic and hemorrhagic risks.

Observation:

  • A 55-year-old female presented with acute pulmonary embolism (PE), acute myocardial infarction (AMI), and cerebral infarction.
  • Initial investigations excluded common hypercoagulable states, arteriovenous shunts, and malignancy.
  • The patient was treated with rivaroxaban (Xa inhibitor) and clopidogrel (ADP receptor inhibitor), along with a beta-blocker and atorvastatin.

Findings:

  • The combined therapy led to initial embolus shrinkage over 10 months, followed by re-expansion.
