Background:

Primary hypothyroidism is a condition characterized by insufficient thyroid hormone production.
Pericardial effusion, an accumulation of fluid in the sac surrounding the heart, can be a rare manifestation of hypothyroidism.

Observation:

A 44-year-old female patient with primary hypothyroidism presented with a massive pericardial effusion.
The patient did not exhibit signs of cardiac tamponade, a life-threatening compression of the heart.

Findings:

Two pericardiocenteses were performed, with the effusion resolving as the patient's hypothyroid state improved.
Despite achieving a euthyroid state, the patient remained hypertensive, necessitating antihypertensive medication.

Implications:

This case highlights the potential link between hypothyroidism and pericardial effusion, emphasizing the importance of thyroid hormone replacement for resolution.
Persistent hypertension in the context of hypothyroidism suggests multifactorial etiology or independent mechanisms requiring continued medical management.
