Summary

Atenolol, a beta blocker, may cause cutaneous vasculitis, a rare skin reaction. This case report, supported by testing, adds atenolol to the list of beta blockers linked to this side effect.

Background:

  • Cutaneous vasculitis is an inflammatory condition affecting skin blood vessels.
  • Beta blocker medications are widely used for cardiovascular conditions.
  • Adverse drug reactions can manifest as various dermatological conditions.

Observation:

  • A patient developed a skin rash consistent with cutaneous vasculitis.
  • The onset of the rash coincided with the initiation of atenolol therapy.
  • Other potential causes for the vasculitis were considered and ruled out.

Findings:

  • A causal link between atenolol and cutaneous vasculitis was suspected based on clinical presentation.
  • The indirect rat mast cell degranulation test yielded positive results, supporting the drug-induced etiology.
  • This represents the first reported case of cutaneous vasculitis specifically attributed to atenolol.

Implications:

  • Atenolol should be recognized as a potential cause of drug-induced cutaneous vasculitis.
  • Clinicians should consider atenolol when evaluating patients with vasculitis, especially those on this medication.
  • This finding expands the known spectrum of adverse reactions associated with beta blocker therapy.
