Summary

For general surgery lasting at least one hour, an initial sufentanil dose of 1.0 microgram/kg is optimal. This dose reduces the need for repeat administrations compared to lower doses, without causing significant adverse effects like hypotension.

Area of Science:

  • Anesthesiology
  • Pharmacology

Background:

  • Sufentanil is commonly used in cardiac surgery at high doses (up to 15 mcg/kg).
  • Smaller doses are anticipated for general surgery, necessitating dose-finding studies.

Purpose of the Study:

  • To determine an optimal initial sufentanil dosage for general surgery procedures lasting approximately one hour.

Main Methods:

  • 62 patients received anesthesia with sufentanil (0.5, 1.0, or 1.5 mcg/kg initial bolus) and etomidate.
  • Repeat sufentanil doses (10 mcg) were administered as needed based on physiological stress indicators.
  • Atracurium was used for intubation and muscle relaxation.

Main Results:

  • The 1.0 mcg/kg initial sufentanil group required significantly fewer repeat doses compared to the 0.5 mcg/kg group.

Related Experiment Videos

  • Increasing the dose to 1.5 mcg/kg offered no additional benefit and resulted in prolonged hypotension.
  • The 1.0 mcg/kg dose demonstrated the most favorable balance between efficacy and safety.
  • Conclusions:

    • An initial sufentanil dose of 1.0 mcg/kg is recommended for general surgery lasting at least one hour.
    • This dosage optimizes anesthetic management by minimizing repeat administrations and avoiding excessive hemodynamic depression.
