Trachea01:22

Trachea

6.1K
The trachea, commonly known as the windpipe, is a vital part of the human respiratory system. It serves as a passageway for air to travel between the larynx and the bronchi, allowing oxygen to reach the lungs. Let's explore its anatomical features, dimensions, layers of the tracheal wall, associated muscles, and the functions of its parts.
Anatomical Features:
Location: About half of the trachea is situated in the neck, anterior to the esophagus, and extends from the larynx (at the level of...

Tracheostomy: Procedure and Tubes01:28

Tracheostomy: Procedure and Tubes

5.6K
A tracheostomy is a surgical procedure that creates an artificial opening into the trachea, typically at the second or third cartilaginous ring level. This opening allows the insertion of a tracheostomy tube, which can replace an endotracheal tube, provide mechanical ventilation, bypass an upper airway obstruction, or remove accumulated tracheobronchial secretions.
Tracheostomy tubes can be made of semiflexible plastic (polyurethane or silicone), rigid plastic, or metal, and they come in...

Esophageal Perforation-I: Introduction

829
Esophageal perforation is a severe medical condition characterized by a breach in the integrity of the esophageal wall. This breach can occur due to various factors such as trauma, medical procedures, or underlying diseases. When the esophageal wall is compromised, it allows food, fluids, and digestive juices into the chest cavity or adjacent structures, leading to potential complications and health risks.
The location of esophageal perforation can vary, occurring anywhere along the esophagus....

Tracheostomy Decannulation01:21

Tracheostomy Decannulation

3.0K
Tracheostomy decannulation is a significant milestone in the liberation of mechanically ventilated patients. Despite its importance, there is no universally accepted protocol for this procedure. This demands an evidence-based, individualized approach.
Description of the Procedure
Decannulation refers to the permanent removal of the tracheostomy tube, signaling the resolution of the condition that initially necessitated the tracheostomy. The process requires a well-coordinated interplay between...

Esophageal Perforation-II: Clinical Manifestations and Management01:28

Esophageal Perforation-II: Clinical Manifestations and Management

1.0K
Esophageal perforations manifest in various clinical forms, influenced by factors such as the perforation's cause and location (cervical, intrathoracic, or intra-abdominal), the extent of contamination, and potential injury to adjacent mediastinal structures. The timing between the perforation occurrence and treatment initiation also affects the clinical presentation.

Diverticular Disease of the Colon01:27

Diverticular Disease of the Colon

18
Diverticular disease involves the formation of diverticula—small sac-like outpouchings of the colonic wall—and their complications. It most commonly affects the sigmoid colon due to higher intraluminal pressure and structural vulnerability. It results from structural weakness and increased pressure in the colon, producing pseudodiverticula that may remain silent or progress to inflammation and serious complications.Structure of DiverticulaIn diverticulosis, these outpouchings are...

Summary

Multiple tracheal diverticula, a rare condition, can cause persistent cough. This case report details a 50-year-old male diagnosed with multiple acquired posterior upper tracheal diverticula, who improved with conservative management.

Area of Science:

  • Pulmonology
  • Radiology
  • Gastroenterology

Background:

  • Tracheal diverticulosis is a rare condition characterized by outpouchings of the tracheal wall.
  • Typically, tracheal diverticula present as solitary, right paratracheal air collections on imaging.
  • While often asymptomatic, they can manifest with persistent respiratory symptoms.

Observation:

  • A 50-year-old male presented with a persistent cough, prompting extensive diagnostic evaluation.
  • Thoracic multidetector computed tomography revealed multiple posterior right paratracheal air collections.
  • Flexible bronchoscopy confirmed the diagnosis of multiple acquired posterior upper tracheal diverticula.

Findings:

  • The imaging findings of multiple posterior right paratracheal air collections were consistent with tracheal diverticulosis.
  • Flexible bronchoscopy provided definitive visualization and characterization of the diverticula.
  • The patient's persistent cough was attributed to the identified tracheal diverticula.

Implications:

  • This case highlights the importance of considering multiple tracheal diverticula in the differential diagnosis of persistent cough.
  • Advanced imaging like thoracic CT and bronchoscopy are crucial for accurate diagnosis.
  • Conservative medical management can be effective for symptomatic tracheal diverticulosis.
