Structured home-based exercise significantly improves knee osteoarthritis (OA) function. Early improvements in the Knee Injury and Osteoarthritis Outcome Score (KOOS) predict successful outcomes, not imaging results.

Main Results:

  • Significant improvements observed in 6MWT (P<0.001) and total KOOS (P=0.025), especially in sports/recreation (P=0.022).
  • Sixty percent (18/30) of patients achieved successful clinical outcomes (Patient Global Impression of Change <3).
  • Early KOOS improvement at 4 weeks was the sole independent predictor of successful outcome (OR: 1.115; P=0.047). Imaging parameters did not predict outcome.

Conclusions:

  • Home-based exercise programs effectively enhance function in knee OA patients.
  • Early functional improvements (KOOS) are key predictors of treatment success.
  • Radiographic and SPECT/CT imaging findings do not predict outcomes for home-based exercise interventions in knee OA.
