Bile replacement (BR) in jaundiced patients undergoing external biliary drainage (EBD) improved blood clotting and cellular immunity. This study suggests maximizing BR quantity enhances these benefits for patients with biliary cancers.
Area of Science:
- Hepatology
- Immunology
- Gastroenterology
Background:
- External biliary drainage (EBD) is used for jaundiced patients, but its role is debated.
- Bile replacement (BR) may improve intestinal barrier function after EBD, though mechanisms are unclear.
- This study investigates the impact of BR on immune functions in jaundiced patients.
Purpose of the Study:
- To evaluate the effect of bile replacement (BR) on immune functions in jaundiced patients undergoing external biliary drainage (EBD).
- To assess changes in lymphocyte proliferation and serum diamine oxidase (DAO) activity during BR.
- To compare outcomes in patients receiving BR versus those receiving EBD alone.
Main Methods:
- A prospective study of 15 jaundiced patients receiving oral BR (200 ml/day) for 14 days post-EBD.
- Measurements included lymphocyte proliferation (Con A, PHA) and serum DAO activity.
- A control group of 20 patients with EBD but no BR was analyzed.
Main Results:
- BR improved serum liver enzymes, prothrombin time-international normalized ratio (PT-INR), and lymphocyte responses to Con A and PHA over 14 days.
- No improvements in lymphocyte percentages or DAO levels were observed.
- The control group showed no improvement in PT-INR or lymphocyte responses during EBD alone.
- Higher fractions of BR correlated with significant PT-INR decrease.
Conclusions:
- Preoperative bile replacement (BR) is beneficial for improving blood coagulability and cellular immunity in patients with external biliary drainage (EBD).
- Maximizing the quantity of bile used for replacement appears to enhance these positive effects.
