Summary

Extracorporeal shock wave lithotripsy (ESWL) shows promise for treating gallstones and biliary duct stones. While 40% of gallstone cases were cleared, ESWL effectively fragmented biliary duct stones for endoscopic removal.

Context:

  • Extracorporeal shock wave lithotripsy (ESWL) has been used for urinary stones since 1985.
  • ESWL is increasingly applied for gallbladder and biliary duct stones.
  • This study presents preliminary Venezuelan experience with ESWL for biliary stone disease.

Purpose:

  • To evaluate the efficacy of ESWL in treating symptomatic gallstones and biliary duct stones.

Summary:

  • Fifteen patients with gallstones (10) or biliary duct stones (5) were treated using a Siemens Lithostar electromagnetic shock wave generator.
  • A 40% success rate (gallbladder clearance) was observed for gallstones over 12 months.
  • ESWL successfully fragmented all biliary duct stones, facilitating endoscopic retrieval in 4 out of 5 cases.

Impact:

  • ESWL is a valuable treatment option for selected gallstone cases.
  • ESWL is particularly useful for biliary duct stones when endoscopic maneuvers fail.
  • This technology offers a less invasive approach to managing complex biliary stone disease.
