Summary

Sarcoidosis can mimic birdshot chorioretinopathy with similar choroidal lesions. Consider sarcoidosis in the differential diagnosis for patients presenting with these findings.

Area of Science:

  • Ophthalmology
  • Rheumatology
  • Immunology

Background:

  • Birdshot chorioretinopathy is a posterior uveitis characterized by specific funduscopic findings.
  • Sarcoidosis is a multisystem inflammatory disease that can affect the eyes.

Purpose of the Study:

  • To describe an unusual case of sarcoidosis presenting with clinical features mimicking birdshot chorioretinopathy.
  • To highlight the importance of considering sarcoidosis in the differential diagnosis of birdshot-like presentations.

Main Methods:

  • Observational case report.
  • Review of clinical, laboratory, photographic, and histopathologic data.
  • Funduscopic examination and lymph node biopsy.

Main Results:

  • A patient with sarcoidosis exhibited hypopigmented chorioretinal lesions identical to birdshot chorioretinopathy.
  • Biopsy confirmed noncaseating granulomas, consistent with sarcoidosis.
  • HLA-A29 testing was negative, ruling out typical birdshot chorioretinopathy.

Conclusions:

  • Sarcoidosis can present with choroidal lesions indistinguishable from birdshot chorioretinopathy.
  • Sarcoidosis should be included in the differential diagnosis for suspected birdshot chorioretinopathy.
  • Patients diagnosed with birdshot chorioretinopathy should be evaluated for sarcoidosis.
