Summary

Immediate dental implant bleeding complications, particularly in the mandibular canine region, are often caused by lingual plate perforation. Long implants (≥15 mm) increase risk, requiring airway management and bleeding control for treatment.

Area of Science:

  • Oral surgery
  • Implant dentistry
  • Surgical complications

Background:

  • Postoperative bleeding is a significant complication in dental implantology.
  • Understanding risk factors is crucial for patient safety and successful outcomes.

Purpose of the Study:

  • To review immediate bleeding complications following dental implant placement.
  • Identify high-risk areas, causes, implant characteristics, and effective treatments.

Main Methods:

  • Systematic literature search of Medline (PubMed) and Embase up to May 2014.
  • Included human studies reporting immediate bleeding after implant placement, noting hemorrhage time, implant length, cause, and treatment.
  • Excluded patients on anticoagulation therapy.

Main Results:

  • Fifteen articles met inclusion criteria.
  • The mandibular canine area showed the highest incidence of bleeding complications.
  • Lingual cortical bone perforation, damaging the sublingual artery, was the primary cause.
  • Implants measuring 15 mm or longer were associated with bleeding.
  • Management involved airway securing (intubation/tracheostomy if needed) and hemorrhage control.

Conclusions:

  • Special attention is required for implants in the mandibular anterior zone, especially longer ones.
  • Perforation of the lingual plate is a frequent cause of bleeding.
  • Treatment prioritizes airway security and bleeding control.
