An ischemic stroke occurs when a cerebral blood vessel becomes obstructed, most often by a thrombus or embolus, interrupting the delivery of oxygen and glucose to brain tissue. Because neurons rely on continuous aerobic metabolism, energy failure begins within minutes of reduced perfusion. The region receiving the least blood flow becomes the infarct core, an area of irreversible cellular death. Background:

  • Cerebral infarction causes detectable changes in diffusion tensor imaging (DTI).
  • Conventional magnetic resonance (MR) imaging may also reflect these post-infarction alterations.

Purpose of the Study:

  • To investigate if texture analysis (TA) of conventional MR images can detect cerebral infarction changes.
  • To compare TA findings with diffusion tensor imaging (DTI) parameters.

Main Methods:

  • Conventional MRI and DTI were performed on 30 patients with first-ever right-hemisphere infarction in the chronic phase.
  • Fractional anisotropy and mean diffusivity (DTI) and texture parameters were calculated.
  • Interhemispheric differences and correlations between DTI and TA were analyzed.

Main Results:

  • DTI showed reduced fractional anisotropy and elevated mean diffusivity in the lesion and surrounding areas.
  • TA revealed decreased homogeneity and increased complexity in the lesion and ipsilateral centrum semiovale.
  • Correlation between DTI and TA was observed in the ipsilateral mesencephalon.

Conclusions:

  • Texture analysis (TA) can aid in identifying infarction-related changes in conventional MR images.
  • TA detects alterations beyond the primary lesion site, indicating widespread effects.
  • Infarcted brain areas exhibit more heterogeneous and random texture compared to healthy tissue.
