Summary

Flicker-defined form (FDF) perimetry effectively detects early glaucoma, but standard automated perimetry (SAP) is better for advanced visual field loss. FDF shows promise for diagnosing beginning glaucoma.

Background:

  • Glaucoma diagnosis relies on detecting progressive visual field loss.
  • Standard automated perimetry (SAP) is the current gold standard for visual field testing.
  • Newer perimetric techniques are needed to improve early glaucoma detection.

Purpose of the Study:

  • To evaluate flicker-defined form (FDF) perimetry's ability to identify functional vision loss in early glaucoma.
  • To assess the dynamic range of the FDF stimulus in glaucoma patients.

Main Methods:

  • FDF perimetry and SAP were conducted on 60 healthy subjects and 111 glaucoma patients at identical locations.
  • Glaucoma patients were categorized into preperimetric, borderline, early, and advanced stages based on SAP results.
  • Statistical comparisons of defect values and receiver operating characteristic (ROC) curves were performed.

Main Results:

  • FDF-mean defect (MD) values were significantly elevated in preperimetric, borderline, and early open-angle glaucoma (OAG) groups.
  • The percentage of patients with abnormal FDF-MD increased with glaucoma severity (27.3% to 87.9%).
  • Ceiling effects in FDF testing correlated strongly with visual field loss, and FDF could not distinguish SAP losses exceeding 5 dB.

Conclusions:

  • FDF perimetry demonstrates potential for detecting early-stage glaucoma.
  • For patients with SAP-MD exceeding 5 dB, conventional perimetry is recommended due to its wider dynamic range.
