Predicting breast conservation surgery (BCS) success after neoadjuvant chemotherapy (NCT) is crucial for operable breast cancer patients. This study developed a novel model using clinicopathologic factors to accurately forecast BCS achievement and tumor size reduction.

Background:

  • Accurate prediction of breast conservation surgery (BCS) success post-neoadjuvant chemotherapy (NCT) is vital for treatment planning in operable breast cancer.
  • Deciding between upfront surgery or NCT requires reliable prognostic tools.

Purpose of the Study:

  • To develop and validate predictive models for achieving BCS and tumor size reduction to ≤ 3 cm after NCT.
  • To identify clinicopathologic factors associated with BCS and favorable tumor response to NCT.

Main Methods:

  • Retrospective analysis of 513 women with stage II/III breast cancer who underwent NCT and surgery.
  • Assessment of clinicopathologic factors for predicting BCS and tumor size reduction.
  • Development and validation of nomograms in an independent cohort.

Main Results:

  • BCS was achieved in 50.1% of patients; 42.2% had tumors reduced to ≤ 3 cm post-NCT.
  • Factors predicting BCS included smaller initial tumor size, lesion-nipple distance, absence of suspicious calcifications, and single tumor.
  • Factors predicting residual tumor ≤ 3 cm included negative estrogen receptor, smaller initial size, higher Ki-67, and absence of in situ component.
  • Developed nomograms showed high predictive accuracy (AUCs 0.800 for BCS, 0.777 for tumor size ≤ 3 cm).

Conclusions:

  • A novel predictive model incorporating key factors for BCS and tumor response post-NCT was established.
  • This model aids in personalized treatment decisions for breast cancer patients considering NCT.
