Summary

Bariatric surgery can cause nutritional optic neuropathy (NON), a vision impairment. Prompt vitamin B12 treatment effectively resolved NON symptoms in a patient post-sleeve gastrectomy.

Observation:

  • A 34-year-old female developed bilateral NON 10 months after sleeve gastrectomy.
  • Symptoms included decreased visual acuity, visual field loss, and abnormal visual evoked potentials.
  • Laboratory tests revealed a low serum vitamin B12 concentration (161 pg/ml).

Findings:

  • Treatment with daily intramuscular vitamin B12 injections (1000 units) was initiated.
  • Within one week, serum vitamin B12 levels increased over threefold.
  • Complete resolution of bilateral NON symptoms was observed.

Implications:

  • The rising number of bariatric surgeries may increase the incidence of NON.
  • Long-term ophthalmological follow-up is crucial for patients undergoing bariatric procedures.
  • Proactive vitamin and microelement supplementation is recommended post-surgery.
