Background:

  • Chronic rhinosinusitis (CRS) often necessitates revision surgeries.
  • Fenestration of the sphenoid sinus can be challenging in revision cases.
  • Image-guided surgery (IGS) is increasingly adopted in sinus procedures.

Purpose of the Study:

  • To evaluate the efficacy of IGS in sphenoid sinus fenestration.
  • To compare IGS-assisted revision functional endoscopic sinus surgery (FESS) with non-IGS techniques.
  • To assess the impact of IGS on surgical outcomes in revision CRS patients.

Main Methods:

  • Retrospective study of 51 CRS patients undergoing revision FESS with IGS.
  • Comparison with 30 CRS patients who had revision FESS without IGS.
  • Analysis of sphenoid sinus penetration and fenestration rates.

  • Sphenoid sinus fenestration rates were 91.2% and 91.3% with IGS.
  • Fenestration rate was significantly lower at 68.6% without IGS (p = .004).
