Coarctation of the descending thoracic aorta, a rare condition, often presents with severe hypertension. Surgical bypass offers excellent recovery results for this aortic narrowing.

Observation:
- Three cases of descending aorta coarctation were surgically treated between January 2012 and December 2013.
- The median patient age was 19 years, with a median follow-up of 16 months.
- Hypertension was the predominant clinical presentation, diagnosed via computed tomography angiography.
Findings:
- Two patients underwent aorto-aortic bypass, and one received a subclavian-descending aorta bypass.
- Excellent recovery was observed, marked by a reduction in antihypertensive medications and restored distal pulses.
Implications:
- Middle aortic coarctation is a rare but serious condition requiring prompt management.
- Etiologies are diverse, including congenital, acquired, inflammatory, and infectious factors.
- Surgical bypass remains the gold standard for repairing mid-aortic syndrome, addressing life-threatening hypertension complications.
